Admission Requirements Admission Procedure
Pre-School Admission Requirements   Grade School and High School Admission Requirements

Age Requirement
  • NURSERY - 3-4 years old
  • PREPARATORY 1 - 4-5 years old
  • PREPARATORY 2 - 5-6 years old
Applicants must submit the following:

1. Original photocopy of the following:
  • Birth Certificate
  • Baptismal Certificate
  • Parent's Marriage Contract
  • Medical Certificate
  • Immunization Record
2. Two (2) recent “2x2” pictures.
 
Admission is evident by the applicant's commitment to abide by the rules and regulation of La Purisima Concepcion Academy

Applicants must submit the following:

1. Form 138, with the certificate of eligibility duly accomplished and
    signed by the Principal.
2. Original and photocopy of Birth and Baptismal Certificate.
3. Certificate of good moral character from the Principal of the
    previous school attended.
4. Two (2) recent “2x2” pictures.
Criteria for Selection of New Applicants
a. Academics – good academic performance from former school.
b. Entrance Test Result – passing mark in the entrance examination.
c. Conduct – At least a grade of 80% in conduct and with NO
    DISCIPLINARY RECORDS.
